My car shakes violently?

After recieving an oil change and tire rotation yesterday, my car began to shake violently when approaching 45mph. Whether I am accelerating or decelerating when ever the speedometer hits 45 mph the car begins to shake hard. The longer it stays at 45 the harder it shakes. I have to either speed up past 50 mph or slow down to under 40 mph. It has shoke in the past on occasions. Mainly if I was coming off the interstate slowing down very rapidily. Any ideas of what the problem could be and how serious is it?My car shakes violently?The tech who rotated your tires may have inadvertently knocked one of your wheel balance weights off.
